Just need some suggestion if the 21 days JR pass will work me, I am travelling to below cities. I know that I have to buy a separate 3 day pass for Hakone but otherwise will it work everywhere.
Can somebody recommend if the below travel to different cities, is how it should be done.
Tokyo 4 nights
Hakone/Mt Fuji 2 nights
Takayama 2 nights
Kanzawa 2 nights
Kyoto 5 nights
Hiroshima 2 nights
Osaka 3 nights
Tokyo 2 night

Look forward to hear from you all.

you still are a little short. But it is not hard to make up the difference with some day trips such as visiting Himeji (with Japan's best castle) from Kyoto or Osaka.
Since you are already seeing Tokyo, you can largely skip Osaka during the day. After the temples of Kyoto close down around 5 PM, you can zip over to Osaka for the evening. At night the city comes alive and has some great places to see, such as Dotonbori, plus the night views from the Umeda Sky Bldg and Abeno Harukas Bldg are wonderful.
On another evening, you could also go and see Kobe.
Instead, use that time to see Himeji, and don't miss Nara either.

That said, you would still get far greater savings by going to Hakone as a side trip from Tokyo, and completing all your long distance travel within 14 days on a 14 day pass.
You could still take numerous day trips from Tokyo for less on the Tokyo Wide Pass.

If going through Narita, the cheapest way is the 1000 yen bus.
