We plan to spend our vacation in Tokyo from October 15 to 21, 2016. Among others, we have this itineraries:
1. overnight to Mt. Fuji. from Narita Airport. What transpo can we use that is covered by JRPass from Narita Airport to Kawaguchiko Station? and from Kawaguchiko Station to Yoyogi station the next day?
2. one-day trip to Hiroshima and back to Tokyo the same day. Is the loop-bus / hop-on/hop-off bus in Hiroshima covered by JR Pass? and whats the last trip of Shinkansen from Hiroshima to Tokyo?

Your idea of going to Hiroshima by train as a day trip is very ill-advised. On a JR Pass you'd sure get your money's worth (and yes the loop bus is covered), but one way is about 5 hours, and the last train back would leave about 5:45 PM. Ten hours on a train for perhaps 5 hours in Hiroshima would be a monumental waste of time.
Instead, why not fly? You can be down there in just 80 minutes, and ANA's Experience Japan Fare is very low.

You'd have at least double the time to see Hiroshima, and there are many good places.
Do you have any suggestion? Instead of Hiroshima, what City can we travel from Tokyo?

But I suggest you consider going to see Kanazawa for a day - you can get there in 2½ hours by bullet train, so that still leaves a fair amount of day time to see the city, it also has an atmosphere like Kyoto, and it has Japan's best historical garden.
Another option is going to see Sendai - you can get there in 90 minutes, and the JR East Tohoku Area Pass

costs considerably less than the full JR Pass. There are actually many places you could see with it.
