My mother doesn't have a last name in her passport. She just has first and middle name. If I write "first name" in place of "last name" (as there is no division) will that cause any issue? For e.g. If I write "FirstName MiddleName" where it is specified to write "LastName FirstName", how will be printed on the JR pass?
Kindly let me know and help me. Need to order the JR pass ASAP.

Please just enter her first name followed by her middle name on the website. As long as the names that are printed on the voucher match those in the passport (the name order is not important, but be sure the spelling is correct!), there will be no issues when it comes to exchanging the voucher for the JR Pass itself.

I have a similar question. I entered my last name and my name in the form, but in my passport my middle name is also shown.... would that cause me trouble when exchanging it for the pass? I have already recieved it and noticed it just now. Both my last name and name are correctly spelled though.

No, middle names are not required by JR. As long as your first name and surname are on the voucher and spelled correctly the voucher will be accepted!
