I'll activate my pass on Oct 19th to move from NRT airport to Tokyo. From there I'll visit Kyoto Osaka an then coma back to Tokyo for a few extra days.
If I buy the 14 days pass, my last day wouldn't be covered to travel from Tokyo to NRT airport. Should I just pay for regular tickets that day? How expensive is that compared to just do the 21 days Rail Pass?

If you are only going to Kansai for a few days, then the better deal would be the japanican ticket for you.

You can use the Nozomi train with it, getting you there 30 minutes sooner, and it also includes a free day of local transport. Even if you buy regular tickets to see some other cities in the area (Osaka, Nara, Kobe), it still comes out cheaper than the 7 day JR Pass.
As for getting to Narita cheaply, you can always use the 1000 yen bus.

--Not necessarily. As I said, if you go Tokyo-Kyoto round trip, which is going to 2 different regions, you still don't make a rail pass pay off. It is all up to how far you travel - not number of regions, prefectures, number of trips, etc.
I can't advise much until you decide how many days you want to spend in what area. There is enough in the Tokyo area for all your time with the city + day trips, and the same for Kansai seeing Kyoto, Nara, Osaka, Kobe, Himeji, and if you want, Hiroshima & Miyajima, etc. You have to divide your time according to your interests.
