Osaka and Nara are only about an hour away, and a single costs roughly 800 yen. Therefore you can get away with purchasing a 7 day pass, but only activating it on day 3. You will then have full use of the pass till your departure.
Thanks for the advise. I'm still struggling whether to visit Nara or Kobe.
In that case, I wont be able to use the JR pass from Kansai Airport to Osaka town. I will be arriving Kansai Airport at 10.25 pm and planning to train to Namba area but I'm not sure whether I would be able to make if last train. Considering that I need one hour in immigration and take luggage. Besides, I also have senior citizens with me so might not be able to rush for that.

What is your best recommendation?
my host suggested me the below since Kansai airport to Osaka town is just a short journey:
"haruka" pass-
http://www.kansai-airport.or.jp/en/access/train/
https://www.westjr.co.jp/global/en/travel-information/pass/icoca-haruka/

you could also consider bus option:

that way you could still keep your JR 7-day pass as suggested by Japan Experience
